How do I get a New Mexico birth certificate?

To order a birth certificate, you must meet one of the following requirements:

• • Be the person named on the certificate (if 18 years of age or older)


• • Be the parent, legal guardian or legal representative of the person named on the certificate


• • Be a government agency or attorney representing the person or the family


• • Have a court order

To order a birth certificate, you can use the VitalChek online service, which is the only way to order a certificate online. You can also order by mail, by completing an application form and mailing it to the New Mexico Department of Health Vital Records Office.

You will need to provide a government-issued photo ID, such as a driver's license or passport, and the required fee.
