How do I get a Connecticut birth certificate?


Requirements


  • • Eligibility: In Connecticut, birth certificates are considered confidential and can only be obtained by the person named on the certificate, their parents, their legal representative, or a government agency with a legitimate need. Procedure


  • • Online: You can order a certified copy of a birth or death certificate online through the State of Connecticut Department of Public Health's Vital Records website. You will need to create an account and provide the required information and payment. Once your order is submitted, it will be processed and mailed to you.

  • • Mail: You can also order a certified copy of a birth or death certificate by mail. You will need to download and fill out a copy of the "Application for Certified Copy of Birth, Death, Marriage or Civil Union Certificate" form, which is available on the Department of Public Health's Vital Records website. You will then need to mail the completed form, along with payment and a photocopy of your identification, to the Vital Records Office.
