How do I get a Washington, D.C. birth certificate?


Requirements


  • • Eligibility: In the District of Columbia, birth certificates are considered confidential and can only be obtained by the person named on the certificate, their parents, their legal guardian or representative, or a government agency with a legitimate need. For death certificates, only immediate family members, authorized representatives, or government agencies with a legitimate need can obtain them.

  • • Required Information: You will need to provide some information about the person whose certificate you are ordering, such as their full name, date and place of birth or death, and the names of their parents. Additionally, you will need to provide your own information and identification.


Procedure


  • • Online: You can order a certified copy of a birth or death certificate online through the District of Columbia Department of Health's Vital Records Division website. You will need to create an account and provide the required information and payment. Once your order is submitted, it will be processed and mailed to you.

  • • Mail: You can also order a certified copy of a birth or death certificate by mail. You will need to download and fill out a copy of the "Application for Certified Copy of Birth or Death Certificate" form, which is available on the District of Columbia Department of Health's Vital Records Division website. You will then need to mail the completed form, along with payment and a photocopy of your identification, to the Vital Records Division office.
