How do I get an American Samoa birth certificate?


Requirements

• Eligibility: Birth certificates in American Samoa are confidential and may only be obtained by the person named on the certificate, their parents or legal guardians, or certain authorized persons or agencies.

• Required Information: You will need to provide some information about the person whose certificate you are ordering, such as their full name, date and place of birth, and the names of their parents. Additionally, you will need to provide your own information and proof of your relationship to the person on the certificate.


Procedure

• In Person: You can order a certified copy of a birth certificate in person by visiting the Office of Vital Statistics and Registry in American Samoa. You will need to provide identification and proof of your relationship to the person on the certificate, as well as payment for the certificate. The office is located in the village of Fagatogo, in the same building as the main post office.

• Mail: You can also order a certified copy of a birth certificate by mail. You will need to download and fill out a copy of the "Application for a Birth Certificate"" form, which is available on the Office of Vital Statistics and Registry website. You will then need to mail the completed form, along with payment and any required identification or proof of relationship, to the office.
